Students Rights and Responsibilities

Students Rights to Ask:

  • Name of Accrediting and Licensing Agency
  • Details about the program, instructional laboratory, physical facilities and faculty
  • Cost of Attendance
  • Institutional policies concerning Refunds and Return to Title IV (R2T4) to students who withdraw.
  • Types of financial assistance available to students; including information on all federal, state, local, private and institutional financial aid programs.
  • Procedures and deadlines for submitting an application for each available financial aid program.
  • How is financial aid eligibility and need is determined.
  • How much of your financial need school is unmet as determined by the school.
  • Explanation of each type and amount of assistance in your financial aid package.
  • Your individual loan information: Fees, interest rates, total amount you have to repay and repayment plans available.
  • Deferment or forbearance for certain defined periods and how to find out if you qualify for them.
  • Steps to take to request deferment or forbearance on Title IV, HEA loans.
  • How they provide written information on student’s loan obligations and information on your rights and responsibilities as a borrower.
  • Who should you contact in the event to request re-evaluation if:
    • A mistake may have occurred
    • Your enrollment or financial aid circumstances has changed
  • How satisfactory academic progress (SAP) is determined and what happens if you fail to make SAP.
  • How failing to maintain SAP affects your eligibility for Title IV, HEA programs.
  • What type of facilities/services that are available to students with disabilities and how to request a reasonable accommodation.

Student’s Responsibilities:

  • Before enrollment understand and consider all the information about the osteopathic medicine program.
  • Complete the FAFSA accurately and submit it on-time as errors may delay or prevent you from receiving Title IV, HEA funds.
  • Understand and comply with all deadlines for applying for and re-applying for aid. Each aid year a student must have submitted new FAFSA information so the school can determine eligibility and awards. Please ask the Office of Financial Aid for assistance if needed.
  • Submit all enrollment and verification documents, corrections and/or new information to the Office of Financial Aid in a timely manner when requested.
  • Information that has changed since you initially applied should be submitted as soon as possible to the Office of Financial Aid.
  • Read, Understand and Keep copies of all forms you signed either electronically or with a wet signature.
  • Know and understand you have to start making monthly payments on your loans after your grace period ends due to not having a deferment or forbearance. Please refer back to your Master Promissory Note about repayment terms or contact the Office of Financial Aid for further clarification.
  • Know by signing your Master Promissory Note (MPN), you are agreeing to repay your student loan back.
  • When you withdraw or leave school, you must complete Exit Counseling online at studentloans.gov and visit with the Office of Financial Aid concerning your current loan debt and the servicers of those loans.
  • Notify the school and any loan servicers of any changes in your: name, address, phone number or attendance status.
  • Know and understand the refund policy and financial charges for attending school.
  • Know and understand the Matriculation Agreement and Policy and Acknowledgement Form that you sign.
  • Know and understand you may be responsible for any Return to Title IV (R2T4), HEA funds returned by the institution on your behalf.
  • Know and understand liabilities could occur when errors are made because of inconsistent information provided by you.  This may result in over-awards of Title IV, HEA funding that you were not eligible for that you may have been advanced or credited to your student account.
